Our social worker can help you navigate the various barriers that may be getting in the way of achieving better health. Whether you are lacking transportation, clothing, food or general support, our social worker can help connect you to resources in our community to meet your needs.
Our community health worker (CHW) assists patients in accessing appropriate resources, coordinates referrals as necessary, provides community and educational support and serves as a community liaison and advocate to bridge the gaps between resources and patients in need.
