About Us
The Ohio State University College of Nursing opened the practice known as Total Health and Wellness in 2012 with a vision to expand primary care and treatment of chronic conditions to the Near East Side of Columbus.
The nurse practitioner-led primary care family practice at The Ohio State University Wexner Medical Center East Hospital provides an interprofessional team approach to integrated physical and mental healthcare for people of all ages.
Total Health and Wellness became a Federally Qualified Health Center (FQHC) in 2019. The FQHC designation recognizes community-based healthcare providers who deliver primary care services in underserved populations. FQHCs are supported by federal HRSA Health Center Program grants, which come through the U.S. Department of Health and Human Services, aimed at expanding access to affordable, quality primary care to vulnerable areas. In 2024, the expanded FQHC presence was renamed to The Ohio State University Total Health Care Center, representing the greater breadth of holistic, interprofessional, team-based care that the practice offered the community.

Our Board: The Total Health Care Center operates under a governing board that includes patients and community members.
The Ohio State University Total Health Care Center is recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (PCMH). PCMH is a model of care that puts patients at the forefront of care. It is designed to build better relationships between patients and their clinical care teams.
