We ask that you provide a valid photo ID, insurance card or proof of income, Social Security card, all current medications and the name and phone number of your previous primary care provider.

We would like to see and care for you and your family regardless of your ability to pay. We have a sliding fee scale that all patients can use, and we accept most health insurances, including Medicare and Medicaid.

Our clinic utilizes various forms of interpretation services, including in-person interpreters and telephone and video conferencing so that we can communicate with anyone, no matter their language.
